A completed Poly application consists of multiple items. Use the checklist below to help keep track of the application requirements.

Middle School Checklist
  • RAVENNA ACCOUNT
    Applicants must have a Ravenna account to access our online admissions portal. Once your account is active, you’ll see more details and may register for events.
  • PRELIMINARY APPLICATION
    Our application opens on September 3, 2024 and includes some background information.
    Deadline: November 22, 2024
  • IN-PERSON INTERVIEW
    For applicants to Grades 5 and 6, interview dates will be available after mid October. Students are required to attend a small group interview at one of the available times. For applicants to Grades 7 and 8, interviews are based on space availability and are by invitation. The interview will include a short writing assessment. Parents/guardians will interview with a member of our admissions committee while their student is in the group interview.
  • IN-PERSON or ONLINE INFORMATION SESSION
    Join us for an information session to learn more about our program and admissions process.
  • COMMUNITY EVENTS (OPTIONAL)
    We offer a variety of opportunities to connect with our Poly Prep community members.
  • STUDENT WRITING SAMPLE
    Please upload a sample of written work completed by your child as a school assignment during the current academic year. The sample may be an essay, paragraph or response/reflection. We understand that there are a wide variety of ways that written work is assigned and teachers provide feedback. We prefer that the writing sample include the description of the assignment and the teacher comments/feedback/grade if available.

  • PARENT/ GUARDIAN SHORT ANSWER QUESTIONS
    Families are asked to complete the ISAAGNY Parent Statement Form.

  • ISAAGNY SCHOOL COMMON ESSAY
    Students will be required to respond so several chosen prompts to complete short essays that will be shared with other schools utilizing the same form.
    Deadline: January 9, 2025
  • TEACHER RECOMMENDATIONS
    Poly Prep requires Math and English teacher recommendations. If both subjects are taught by one person, that teacher must complete both recommendation forms for your child. We strongly recommend that you request the recommendations after October 15 to give the teacher enough time to get to know your child and complete the form before it is due.

  • TRANSCRIPTS
    Download the Transcript Request Form in Ravenna and print or email it to the records office at your child’s current school after December 1.

  • ISEE PRIMARY or SSAT
    Testing is required for admission into all MIddle School grades this year. You can read more about our new policy here. Poly will superscore multiple test score submissions. Families are welcome to submit scores for either the SSAT or ISEE.

  • SUPPLEMENTAL DOCUMENTS
    Any other supplemental documents as detailed on the application.
